2. Comprehensive in-home clinical assessment
We visit you in the comfort and privacy of your home for a thorough assessment. Rather than rushing, we look at the entire picture: your tissue viability, blood glucose control, mobility, nutrition, and overall wellbeing. You will always see the same consistent nurse on every visit, ensuring vital continuity of care and the ability to detect subtle healing progress early.
3. Tailored care plan and digital visit notes
Together, we establish a personalised care plan detailing the necessary treatments and visiting schedule. Following each appointment, we produce secure digital visit notes and wound progress documentation. These can be shared directly with you, family members, or your GP, ensuring full transparency without paper clutter or confusion.
